I was looking online the other day and found the below Behavior Based Safety Observation Form. While it is not automated it is an example for a manual BBS system and could help get your ideas flowing. The exact items you may have employees observe shoudl be based on your data and not these.
This could easily be used as an outline that could be taken to your team to customize.
